We understand some families may not be able to pay the signing on fees in full in the current climate. We don’t want this to prevent your son/daughter playing football. Therefore, if you wish to discuss alternative arrangements please email our treasurer on hcarthy@hotmail.com.


YOUTH PLAYER MEMBERSHIP 2020/21 SEASON

League rules require a recent photo of the player being registered. Please upload 1 file only. File type accepted .jpg.
Does your child have any known health needs? E.g. Diabetes, asthma, epilepsy, allergies. If YES, the Medical Declaration section of this form (below) must be completed in full.
This form is to be completed by the legal carer and child at the start of each season. It is recommended that this form is completed and signed by the legal carer and player at the same time. Legal carers are responsible for informing Faringdon Town Football Club of any changes that occur. It is important that you fill in this form as fully as possible. Failure to tell us things could mean that the safety and welfare of your child are compromised. Faringdon Town Football Club cannot be held responsible if the information has not been shared correctly.
What does the Club need to do to help keep your child well e.g. administer planned medication/call ambulance/give snacks? Please be very specific. Do Club members need any medical training other than First Aid to care for your child? If yes please specify. Does your child have any access needs? If yes please tell us what we need to do to help him/her.
I give permission for my son/daughter to participate in Faringdon Town Football Club’s events and agree to the conditions outlined on this form. I accept that it is my responsibility to inform the Club directly of any changes to details recorded in this form. In the event that my son/daughter is injured whilst playing football and I cannot be contacted on the number supplied, I hereby give my consent for my child to receive medical attention.

FARINGDON TOWN FC CODE OF CONDUCT

As coaches, parents/spectators and players, we are all responsible for promoting high standards of behaviour in football. Please play your part and observe the following code of conduct;

COACHES
When working with players, I will;

  • Place the well being, safety and enjoyment of each player above everything
  • Refrain from, and refuse to tolerate, any form of bullying
  • Develop mutual trust and respect with every player to build their self-esteem
  • Encourage each player to accept responsibility for their own behaviour and performance
  • Ensure all activities are appropriate for the players’ ability level, experience, age and maturity
  • Agree to follow all COVID-19 safety procedures required by the club

PLAYERS
When playing football, I will;

  • Arrive on time with correct equipment including shin-pads and get ready quickly
  • Not wear jewellery
  • Always try to the best of my ability even if losing or the other team is stronger
  • Listen and respond to what my coach/manager tells me
  • Encourage my teammates at all times
  • Never be rude to my teammates, the other team, the referee, spectators or my manager/coach
  • Display high standards of behaviour in both training and at matches
  • Do what the referee tells me
  • Support my teammates from the side if I am not playing
  • Not use or tolerate inappropriate language
  • Never engage in any form of bullying
  • Play fairly, listen to the referee, never cheat, complain or waste time
  • Shake hands with the opposition at the end of a game
  • Appreciate the importance of striving to win, not just the winning itself
  • Accept victory and defeat equally
  • Agree to follow all COVID-19 safety procedures required by the club

PARENTS/SPECTATORS
During coaching sessions and matches, I will;

  • Appreciate my child’s involvement in playing football because they enjoy the game, not to please me
  • Ensure my child arrives on time with the correct equipment (including shin-pads) and is collected on time
  • Remain outside the field of play
  • Never engage in, or tolerate offensive, insulting or abusive language or behaviour
  • Always respect the coaches decision during matches
  • Applaud effort and good play as well as success
  • Refrain from shouting instructions at players to allow them to learn from their own decisions
  • Not be seen drinking alcohol during training sessions or matches
  • Not smoke or vape near to the pitch or training area
  • Pay my fees on time
  • I understand that breaches of the code may result in action being taken by the organisation running the sessions.
  • If you are unhappy about any aspect of your child’s training or matchday experience, please talk to the team manager. If this discussion does not address the issue(s) you have raised then please contact the club Welfare Officer or Head of Youth Development
  • Agree to follow all COVID-19 safety procedures required by the club
